I am wondering if it is typical for track lighting to be fed directly with NM cable without a J-box? Is this a standard track part I can order? I looked at the catalog yesterday but it wasn't totally clear to me.

I think that most track end fittings have a 1/2" KO in the back to allow the entry of a cable without the need to use a box and canopy. This Juno fitting (R38) says that it can be entered from above.
I've always heard them called a Live End. Same width as the track and about 1 1/2" long. They are not easy to make up. I used to buy stranded MC and a particular connector to made the job easier.
Yes they're tight especially with #12 conductors. With #14 solid conductors there's little problems. Just remove the screws completely and fold them down into place. Then put the screws back in.
