Is the MDP on the primary or secondary side?

If you are talking about the secondary overcurrent protection take a look at 240.21(C) for transformer secondary conductors.

It can be more difficult...

Some localities require a transformer primary disconnecting means to be within sight of the transformer.

A transformer does not require a "primary disconnecting means". A transformer requires a primary overcurrent device that does not need to be located "within sight".

Yes per NEC. Since the answer was nailed, I was just pointing out to watch out for local codes...

Some Localities (including Chicago) require a primary disconnecting means, within sight of the transformer.
