The size would be based on your derived pahse conductors and termination would be determined by 250.30(A)(7).
Have you taken a look at 250.30 at all ?

As Augie points out, read through 250.30, it will answer your questions.

I am installing a 3 phase, 150 kva 120/208 transformer, what size ground wire will I need, and do I have to run . It to the building ground?

Can you provide the conductor sizes and the OCPD sizes?

Welcome to the forum. :)

this sounds like a typical transformer install I will assume that the Primary is 460 volt ( 200 amps) and the secondary is 120/208 volt (400 amps). If this is so then the typical install is a # 2 copper ground and dont forget to ground XO and the transformers case as well. Also check to see as most manufacturers ask you to loosen the bolts that hold down the core of the unit a 1/2 turn or so after instalation.

How did you come up with a #2? Are you assuming parallel #3/0's secondary conductors?
