Panel LP! is serving a remote transformer. Does the transformer need a primary disconnect adjacent to it?
 
If it's in the same building, no.
A lockable means on a remote disconnect may be required depending on which Code cycle is in use.

Not necessarily. 2011 NEC

450.14 Disconnecting Means. Transformers, other than
Class 2 or Class 3 transformers, shall have a disconnecting
means located either in sight of the transformer or in
a remote location. Where located in a remote location,
the disconnecting means shall be lockable, and the location
shall be field marked on the transformer.

I agree with Gus, what code cycle are you working under?

2011 NEC section 450.14 now requires a disconnecting means for transformers.

Here is what that section says;

450.14 Disconnecting Means. Transformers, other than
Class 2 or Class 3 transformers, shall have a disconnecting
means located either in sight of the transformer or in
a remote location. Where located in a remote location,
the disconnecting means shall be lockable, and the location
shall be field marked on the transformer.

Panel and transformer are in the same building but different floors.

Then it's your choice, lockable disconnect at the origin of the feeder and a sign or a disconnect at the transformer.
