I do not think a TVSS is a code issue. You install them when you decide the equipment served warrant the expense.

The TVSS on the primary will also protect the secondary side if the voltage transient originates from the primary (aka utility).
If there is a voltage transient originating on the secondary then the TVSS on the primary will not provide much protection, because of the impedance of the transformer.
Generally the TVSS are only located primary as close as possible to the incoming utility lines.
