Two conductors under same lug on line side of MB

I thought it's a violation to tap two conductors under one lug screw on a main breaker. In this case, a blue #12 conductor is under the C phase lug of the line side of 200 amp breaker with 3/0 conductor. This #12 goes back outside to the meter base. Also, this is a 240 Delta service and this C phase is the high leg and identified blue instead of required orange markings. This also passed AHJ inspection with sticker on cover. Just wondering if this is common or if you came across this scenario before. I have attached a picture. Thank you.
 
I tried several times to attach pic but it won't upload.

I just don't know how it passed inspection. I'm thinking the inspector probably just gave the contractor a sticker to slap on panel cover without looking at it.
 
Inspectors are human. It maybe wasn't seen for some reason (which covers a lot of possibilities) or that inspector doesn't know any better.

There really isn't a code section to quote that directly addresses this problem, it is a 110.3(B) violation and one must look at the listing and rating to determine it wasn't installed as intended by the product instructions, though this is going to be against the instructions for nearly every similar connection device out there unless it states right on the device it can take more then one conductor (and will state what sizes those conductors may be as well).

Around here the wild leg terminates to C phase on the Meter and B phase on the interior disconnect or panel.
