I thought it's a violation to tap two conductors under one lug screw on a main breaker. In this case, a blue #12 conductor is under the C phase lug of the line side of 200 amp breaker with 3/0 conductor. This #12 goes back outside to the meter base. Also, this is a 240 Delta service and this C phase is the high leg and identified blue instead of required orange markings. This also passed AHJ inspection with sticker on cover. Just wondering if this is common or if you came across this scenario before. I have attached a picture. Thank you.
