It is also not accessible without tools for digging or strong fingernails, but the code may not take issue with that since the building finish is not involved.

Hand holes can be covered with rocks or bark mulch.

I wouldn't but some loose mulch like shown in the photo is IMO not a problem.

That fitting looks to be 2" or larger. That would be buried about ~6". A tad much IMO.

It was probably above grade when installed and the mulch was added later, not even worth mentioning IMO.

That's almost exactly what I was about to say. Couple of scoops with a shovel to get the cover off an LB is a good day compared to some pole lights and old buildings I have worked on.

I believe it is a problem. Water can most certainly get into the equipment in this case.
