QO breakers are shipped with a compound on their jaws. This should be replenished whenever the breakers are moved. Schneider Electric used to sell small tubes of PJC.

Yeah many do not realize this. Many glaring problems on IR scans are plug on type breaker connections.
The #1 safety enhancement to that setup would be to change the duplex to a GFCI (below that panel).

I don't see how there isn't a fault between the phase conductors as they are very close.
