If the KOs don't line up what about using fender washers behind the box to bring it out with screws through them naturally. To me it seems ok if it's only 1/4-3/8 of an inch or so

Instead of trying to figure a way around it, why not just do it the right way instead of like a hack?

Offset nipples are something that every EC should have on their trucks. I personally have never used or seen that box spacer. Never had a need to put anything that close. If I need to put two or more boxes in a row I use straight nipples and 4 locknuts.

Nothing wrong with using long enough screw and stacking 3 inches if washers to space it, though in that sort of situation I often cut a short piece of EMT or PVC to use as a spacer. Easier to handle one piece instead of many pieces when assembling.

Nothing wrong with the box spacer. faster than close nipple and 4 locknuts, plus close nipple needs bushings. Chase nipple is too close for some raised covers. Often the choice is dependent on what you have. I only keep a few those box spacers as don't seem to need them too often.

Mostly used for surface wiring and maybe a situation where you need 3 or 4 switches but you don't keep any 3 or 4 gang surface boxes on hand, just 4x4's and raised covers, you then can put two 4x4's next to each other with one those spacers between them. I am likely to use an offset nipple for a box next to a panel most the time.
