Has the definition of 'spanner' been updated to include box-end in addition to open-ended wrenches?

Is "Tighten until it's comfortably snug" a valid measure of the correct torque?

How do they come up with the correct torque number anyway? Line up a dozen or more experienced guys with 'calibrated elbows,' have them tighten things, and measure how tight they made things??
