Looking at the other pictures.
Other than maybe some connectors may be missing, I do not see code violations. Maybe some neatness issues, but not code.
Being neat only counts when one cares.
Identification of the white conductor as an ungrounded conductor installed on the breaker may not have been required when it was installed...that is a fairly new requirment.
If it helps, I know the house was built in '91.
Lets start with Picture #1

1. Access to the disconnect
2. Open conductors hanging to the right of the disconnect - this is more of a local code.
3. Improperly supported cables to the 411 box
4. bending radius of the cable entering the box that is terminated to the disconnect
5. identification of the disconnect...unless it is easy to determine what it is disconnecting. that is difficult to determine from the picture posted.
6. Also hard to determine from the picture, but it may be possible the connector for the cable secured to the 411 box is not properly sized.
7. There is an unoccupied fitting to the box terminated to the disconnect

looks like it. I see a red and yellow wire in that grey sheath.

While it doesn't snow it the pix, there's a green and black in there too. Yep....POTS right through the panel! I couldn't see any markings on it, and it sure felt like 4-wire, so I skinned a little to satisfy my curiosity.
You taped it back up, right?
