Wire nuts for fixtures

Ok silly question but aren't many of mine.. But they do generate good conversation between members. It's a three part question. If you 1x4 and were using 12/2 mc cable.

1. You could have cable coming out of one and or both ends?
2. You would have three wire nuts for each cable if it was not an EM fixture.
3. If you get the fixture pre-tailed do they(manufacture )connect the wires in the fixture or just leave them hanging for the EC to connect in the field ?

Pre-tailed fixtures are setup so that you only need to splice the free end of the tail to energize the fixture. Fixtures without tails would typically require 3 splices within the fixture (H, N, EGC). Lately we've been installing mostly Lutron Eco ballasted fixtures which require 5 wires instead of 3.
