Xfrmr Tap

Have to tap a feeder feeding a transformer. The way it was described to me was they were going to add lugs in the xfrmr and run conduit and wire 10' to a disconnect. Does this make sense? Can you add lugs to xfrmr and if so in my mind your not tapping. Or maybe technically your are!?
 
If the added conductor is protected at or below it's ampacity, you don't have a "feeder tap" Same goes for the supply conductor to the transformer.

If both conductors mentioned above are protected at or below their ampacity - you just have a circuit with multiple loads connected to it.
 
I believe the secondarys of all transformers that require secondary conductor protection to be taps.

I had that thought yesterday also, but OP said:

I think the lugs are getting installed on the primary side.

If so, 240.21(B)(3) does not apply either since the tap is not feeding a tranny.

I am guessing 240.21(B)(1) or(2) may apply.
