Surge Protection Device Failure

We had a surge protection device fail within minutes of installation. I've contacted the manufacturer and am waiting to hear back from them. The system appears to be 480 VAC, 3 phase, with one leg grounded. Any ideas for possible causes for this type of failure?

It's the right voltage rating.

I'm going to guess that you connected the neutral wire to the grounded phase, which put 480v across components expecting to see only 277v.

I suggest asking ICM for the correct corner-grounded connections.
 
My guess is that it is not rated for a corner grounded delta. I bet it sees the ungrounded to grounded voltage as a line to neutral voltage that exceeds the line to neutral rating of 350 volts.
