12 lead 3Phase AC motor

Not sure where to post this but I need help please.

I have this 7.5HP Leroy/Somer 3 phase motor...12 leads...numbered as such from 1 to 12.

I am familiar w/ a standard 9-lead 3 phase, but not this 12 lead.

I am trying to make a temporary 3 phase converter so I can run my small lathe and mill at home.

I will appreciate it if anyone can give me some information on how to wire this thing. I do not have the schematic.

It is multi voltage.. 200/230/460/575/600
