410.30(C) has 3 paragraphs, do you have to comply with all that apply or any one of the 3 that matches the situation?

Does complying with paragraph 2 eliminate the need to comply with 1, and 3?

Pick the one that applies to your installation.
 
The OP must be using the 2005 code since 410.30(C) does not exist anymore. In the 2008 & 2011 it is section 410.62(C)

The installation in question uses a mogul base lampholder and a cord and plug remote ballast mounted on a wall, it is not above the fixture as would be required in 1.
The mogul base bumps it into 2, however since it is cord connected I am thinking both 1 and 2 apply.
I am concerned about the use of 6-15 receptacles on a 30A or 50A branch circuit for HID lighting. There is supplementary protection built into the ballast which is what the reference to 240.5(A) is as well as UL listing requires ballast cores to have thermal protection.
This paragraph 1 I think wants the receptacles above the light. If the receptacle can be down on a wall what is to prevent another type of equipment from being plugged in later on?
