In my area pad mount transformers are everywhere, even peoples front yards. Virtually none of them have fences or need fences assuming the access doors are properly secured and locked.

Exactly and they are designed so very hard for even a electrician to open them. I find it hard to believe a poco man left this unlocked. Someone made a major goof here and needs to be fired. I would get major a-- chewing if i even left a cover off on a live panel on construction site where kids would not be. Fence is not neeed.Maybe more labels are needed.
I seen something just as bad at a foster home for kids last month. Its been fixed now , management had no idea the problem was even there but wanted it fixed.
 
I's a dreadful incident with the worst possible result.
We can all learn this.
The transformer should not, in my opinion, have had exposed live terminals.
Prevention from access to it should have been more secure.
 
I used to do service work at serveral apartment complexes around town, I was always surprised at how many unlocked transformers i would find, apparently the maintence man or whoever would cut the pad lock off to replace current limiters and leave them unlocked.

It's always said when this sort of thing happens no matter what the age especially kids.
My area is the same as I-wires.
Always have seen Poco locks on them.

I posted on this site a while back that the transformer infron tof my house was left wide open. I nearly lost it and stood near by until the POCO showed up 5 min after I called 911. They were embarassed as they should be. I told my wife if I didn't see it I bet someone would have been killed since I live in a town house where lots of kids play on or around these.

I addition to that on July 4th the transformer in my back yard blew up. I mean KA-POW flash and the whole 9 yards. I was on my deck and saw it. Luckly no kids were on it but they saw it..... Now they know not to play on them.

I am guessing that this scared a lot of people due to the number of censored posts. All I can say is that Electricity doesn't care who you are. If you don't know anybetter, ( like I am sure this 5 year old) could not of known that it was no more than a box to play Hide and Seek in. Granted, if it wasn't, it should have been locked. I don't know. God bless the family.:mad:

You right, you have not quoted him. However I am looking at the posts in a threaded format and your post is linked to his post and to no other. He offered social critique and since you commented on social commentary as being off topic I replied to that specific comment, with MXSlick's comment boing the example where social critique may be an appropriate aspect of the problem.
 
The other sad part of this situation is that we are all just guessing at what happened. The news media don't know enough to even ask the right questions, and, due to fear of lawsuits, the folks that know aren't talking. So, the FACTS, that might be educational and helpful, remain hidden.

OK I am not driving now so I can post a bit easier.

I think Slicks comments were addressing that the safeguards provided by the current Code and Utility installation practice does not appear to provide sufficient protection against conduct that became acceptable, or largely tolerated.

I had not even read all of slicks comments, my post had nothing to do with his comments, it was a general statement aimed at all members setting out the rules for this thread.

Which you immediately chose to ignore .... no problem I edited your post.

So after some thought here's a possible practical solution to the very issue I pointed out on my post on page 2:

Most POCOs try to be very viligent on keeping their gear closed and locked, but they have neither the manpower or the crystal ball to ensure that EVERY piece of gear is closed and locked 24/7. SO it is up to the people who are affected by or close to that gear to call it in.


How about using SCADA (System Control And Data Acquisition) and some sort of door interlock switch to alert the POCO when a door or cover has been opened or removed? I don't think it would cost an unwarranted sum of money. Edited to add: I understand from my POCO contacts that a lot of vaults already have alarms for water, high temp., etc, so it couldn't possibly be that difficult to program in a door alarm.)

I think it would provide a backup plan to help prevent a repeat of this tragedy.

But, despite all the technology, keep one thing in mind: You can make a system as foolproof as possible, and along will come a better fool.

Or to put it another way, if vandals want to break into something, all the locks, fences and guards won't stop them.
