I ALWAYS test everything twice, every way possible, phase to phase, phase to neutral and phase to ground, then I test my tester on a known live circuit and retest as noted.

That's something I have failed to do: verify the meter afterwards.

Agreed! I can just imagine pulling that meter and not checking power and then sticking my big metal allen wrench into one of the lugs and getting lit up.
