The box pictured holding the keyless is a 2X4 DEVICE box. This box is not listed for the installation of light fixtures. As outlined in the exception to use this type box or a device extension ring that accepts 6-32 screws the fixture would be required to be mounted on the wall.

So If I mount the switch box on the wall then the porcelain is okay. A little absurd with the weight of a porcelain. There ought to be an exception there.

Some of the keyless fixtures have the terminal screws at a right angle with the box and the screws are exposed after installation on a device box.

I have never come across this situation of exposed screws. I agree that would be an issue but is I wall mount the box the screws would still be an issue. That is just a bad installation.

Don't get me wrong I always use round boxes for lights in the ceiling. I just don't see it as a big issue in the attic for a porcelain.

It is an issue, a code issue...

It's possible that the terminals on a keyed or keyless lampholder may fall ousude the perimeter of a rectangular box.
