Does'nt that come under threaded hubs?

Exception: Where threaded hubs or bosses that are an integral part of a cabinet, box enclosure, or raceway provide a smoothly rounded or flared entry for conductors.

That topmost knockout on the meter can would have been field cut. I wonder if there was a warning sticker prohibiting field cut knockouts in that meter can?

I looked at it again. I see what you mean regarding the C phase unless my vision is going.
 
Regarding the C phase on the bottom.

The person could not make the turn from the raceway directly to the lug.

If you look closely it leaves the lug, heads left then takes a U turn and heads right for the panel with the A and B phase.
