If I land my GEC in a meter main to grounded conductor. Do I have to take 4 wires to my first disconnect or just 3 wires. two hots and grounded conductor thanks

No, the conductors between the meter and the first overcurrent protective device are service conductors. The first panel is the service panel and anything taken from that panel to another panel should be 4 wire
 
Along with Dennis's post, you will still have to install a Main Bonding Jumper in the service equipment whether it is a Main Breaker Panel or Disconnect.

Would it be code Compliant if I ran 4 wire from meter can to panel, and just Isolated the ground from Neutral thanks
 
I found it in the code book. main bonding jumper Shall be used to connect the equipment grounding conductors, and the service disconnect Enclosure to the grounded Conductor. Within the enclosure With each service disconnect, if I run a 4 wire from mater main I would be paralleling the Grounded Conductor

OP said he had a meter main. The bonding is done in it and you run a 4-wire to the house/building panel.

That's what it sounds like to me. The service disconnect is at the meter.

So, if it's a meter main what is this "first disconnect"

Yep, there is confusion in the way it is stated. If there is a meter main then that is the first disconnect so 4 wires to the next panel
