I did not read all of every post at this point, but lets remember with aerial conductors they do not necessarily need to be "insulated". Sometimes they are only required to be "covered".
I don't think the covering on most aerial cable is an insulation type mentioned in 310.104(A), but is still considered "covered"
Electrical insulation by isolation is still necessary, especially when talking grounded conductor and equipment grounding conductor.
