AHJ approved separate service. Can the separate service be supplied by the same transformer feeding the building, or, does it have to be supplied from another source? :?

IMHO, I would think that 700.12(D)(2) would not allow the "emergency service" to be fed from the same transformer.

Another source, if the building transformer failed you would have no emergency service.

I don't see anything in the code wording that would require that there be an additional transformer for an "emergency service". If that was a requirement there would be no need for the words in (D)(1) as there is no way a single service drop or service lateral could be used where there are two transformers. The key to this section is the words; "Where approved by the authority having jurisdiction"

I think you are right, that's the key. And I would think an AHJ would be crazy to approve a second service from teh same xfrormer as an emergency service. If it were me, I would double check with the AHJ and make sure he isn't expecting services from separate transformers.
