exemption(s) to 250.148

Is(Are) there any exemption(s) to 250.148 (i.e. metal junction boxes using NM cable in an attic)? Or is the requirement for all boxes (regardless of accessibility) to be grounded using the methods in 250.148? Thanks, Wayne H.
