Mike, I know the exception.

Looking at my 1990 NEC, at that time the section for ranges and dryers was 250-60


From 250-60(c)
'the branch circuit originates at the service equipment'

If the branch circuit originates anywhere but the service equipment it must be four wire and I think that rule goes way way back.

In the 1996 the section is also 250-60(c)
