Hello what is the NEC code for residential smoke detector separation from walls ?
My understanding it can’t be placed in the first 4” from ceiling and wall , I was told it had to be install with in 12” .

Ok, if you had a vaulted ceiling that was 21’ to the peak, where would you guys put it? It must be serviceable right?

At 21 feet, most designers would be considering beam detectors, but not for residential. You'll need a heck of an A-frame ladder or a Genie lift. See NFPA 72 for more discussion on smoke detectors/alarms on sloped ceilings.
