That might be another one of those 120v panels modded for 240v.

Got that Pic from over at the electrical-contractor.net site in the nostalgia section. Think it's from the early 1900's and there was also a code book reference on the requirements when building your own busbars etc..
Pretty cool stuff, some dude actually built that and installed it at a house and was a normal thing.
Probably get a hershey bar for a nickel in that day after walking 10 miles to a friends house to borrow a school book
