If a 150 kva transformer is prefabbed off-site with Greenfield and conductors terminated for both primary and secondary... would the handling of the equipment during shipping, delivery and installation require the terminations to be cut...restripped and torqued again?

Is there an article or reference material that could provide facts on how many nontorqued terminations fail....for instance during fault events where high short circuit currents flow

I would recommend performing some form of NETA Maintenance/Acceptance Testing on the unit keeping the terminations intact. And since the unit was moved, I would include a turns ratio test.

answer to the first question is of course it would- after determining that the terms are not correctly tourqed. i think cutting and restripping would be another question... plus its a good idea to (shipping damage aside- those whips tend to be man handled) visualy dble check all pre fab stuff...
