Worse case high voltage scenario

There are a number of ways the utility gets its single phase. It may not be a neutral but it is a grounded conductor of a 7200 volt winding.
 
I thought I've seen a utility transformer fed by one primary wire on the top, so where does it pick up the grounded conductor? Also, I've seen a 480D/240y where the neutral is picked up at the secondary. I'm a little confused now.
 
I think the high voltage tap for the grounded conductor on the high side of some of the smaller pole distribution transformers are attached directly to the case.Thats fine just , but the question is should they jumper that case ground to the secondary onto the low voltage neutral?

The secondary terminals are all insulated from the case Line #1, Line #2 and the neutral.

I'm sure there are exceptions to this.
