Does Anything Look Odd with this Service?

Not really sure, but the phase markings above the terminals apprear to be N-3-2-1. Reverse rotation?

Also, there's some 'spare parts' in the far end of the cabinet.
 
I'm with Jim. It could be that the majority of the loads do not require a neutral so a derating was applied. If this is the case I sure hope the building isn't re-purposed without someone looking at the service!
 
Without knowing the service requirements it is not possible to pass judgment on the single 'grey' conductor. If this is a high impedance grounded system, the single grounded conductor is allowed to be run separately from the phase conductors.
 
ok, if that is the neutral, and its not high impedance, and I am counting 9 other conductors, then the neutral does not meet the 12.5% rule.
