I voted yes.
If someone with long hair, tats, beard, piercings etc. gets hired they're on limited duty. They can't be sent to a resi job, OSHA job, office job, bank job etc.
The boss is constantly explaining they are good workers and not on drugs or booze.
If they don't work out the person who hired them, may get fired as well. So, the safe bet is don't hire them.
If you didn't know better would you let Dr. House (TV) work on you?
From our on-line comments, I would hire most of the "no votes". But I've formed my opinion over time, not the first opinion.
Lastly, we dress and groom for the job we want. If someone wants high pay and promotion they should, IMO, play the part.
