subpanel grounding

Need to install a sub panel next to an old 200 amp main panel with no main thats fed with an underground 3 wire system from a rural meter loop on pole.Where do you land the new subpanels neutral and ground on old panel?Do you have to separate old neutral and grounds on old panel?And should there be a main breaker installed in old panel?

Presuming there's only a single neutral/grounding bus, both should land there.

Do you have to separate old neutral and grounds on old panel?
With a 3-wire feeder, there is no way to separate them, nor is there a requirement to do so if compliant when installed.

And should there be a main breaker installed in old panel?
Again, no need if originally compliant.
