I see no ambiguity. (310.4 is pretty clear too)

122(A) is titled "general"
122(F) is a specific application that IMO would over-ride 122(A)

That being said, I think I would be in favor of a change in the code to limit the size of the EGC in these extreme scenarios...but I won't let that impact what I think the code says today.

It does not make sense to have the equipment ground conductor larger than the phase conductor, as the phase conductor will be limited by it's size in regards to the ampacity. Hence, the first paragraph of .122.
